EasyMED is a coordinated medication management program designed to simplify patient medications so patients can manage them more easily.

With EasyMED prescription and Over-The-Counter medications are synchronized to be due on the same day, packaged in convenient adherence strips and delivered right to the patient’s door each month.

 

Our Senior Care Pharmacists & Patient Care Coordinators work with patients, caregiver(s), physician(s), Home Health Agency, and other providers to ensure patients are taking the right medication, at the right dose, at the right time...…..every time!

 

This program is FREE as we do not charge for packaging or delivery.  Patients are responsible only for their copays.  

 

We service the West Virginia counties of Boone, Cabell, Fayette, Jackson, Kanawha, Lincoln, Logan, Mason, Putnam, Roane and Wayne.
